MedPeds Policy on Completing Forms

Our providers will be more than happy to complete forms you need for school, work, FMLA, etc... however, we do charge a small fee for the provider's time if the form is completed outside of your office visit:

• $10 for most forms
• $25 for long or complicated forms, such as forms for FMLA or disability benefits
• $0 for forms for patients with state insurance
• $0 for all patients if the form is completed during your office visit
